Survey Program Links

In the past, the surveys were mailed to families with students having an IEP.  This year, the survey will run online so to minimize the handling of paper forms amidst the Covid-19 pandemic. The NHDOE has created two easy to use links based on what grade your student is currently enrolled: Preschool families and K-12 families.

Once you access the appropriate link, you will see a dropdown menu of Block B districts. You will then choose the Candia School District (SAU15 Auburn, Candia, Hooksett) and complete the survey for each of your students that currently hold an IEP.
