2020 NHDOE Indicator 8 Special Education Family Survey

Just a reminder to those parents with students who currently have an IEP… 
 
This survey will remain open through June 30th, 2020. 

The New Hampshire Department of Education (NHDOE) has launched the 2020 Indicator 8 Special Education Family Survey. The survey is for families with students who currently have an Individual Education Program (IEP).

Survey Program Links

In the past, the surveys were mailed to families with students having an IEP.  This year, the survey will run online so as to minimize the handling of paper forms amidst the Covid-19 pandemic. The NHDOE has created two easy to use links based on what grade your student is currently enrolled: Preschool families and K-12 families.

Once you access the appropriate link, you will see a dropdown menu of Block B districts. You will then choose the Candia School District (SAU15 Auburn, Candia, Hooksett) and complete the survey for each of your students that currently hold an IEP.

If you have more than 1 student with an IEP, please complete a separate survey for each of them.

We ask that each of you complete this survey in a timely manner so that the NHDOE can have the most accurate information about Special Education.
